Different organisations handle country code top level domains (e.g. Nominet handles .uk domains).
Registrars usually allow users to register domains that hang off top level domains and the country code top level domains for the country they are based in (e.g. Easily handle .uk domains as well as .com/.net/.org/etc)
.com is commerical .net is internet. The American Government regulates the domain names (ICANN) so yes, you have to buy one. I recommend if you are going to buy one, you buy it from 1and1.com as they are easy to work with in my experience and cheap. There are different domain names for countries. ex. .uk (United Kingdom) .ch (China) .tw (Taiwan) .mx (Mexico) .ca (Canada) and the list goes on…
