I can put together the form and all but i cant get it so that submits the info to my email. I read something about hafting to have a CGI and im testing it on Geocities but the official site that i am building for a friend is going to be on GoDaddy.com.

You need to post the form data to a script that emails it to yourself.
The script can be perl or php or a heap of different things.
Or you can find a website somewhere that allows you to post the data to it and it has a script that runs and emails it to you.
If you’re okish at programing, I’d suggest looking at PHP. Heaps of tutorials on the web about how to email yourself the stuff from the form. As long as your host supports php and email.
Buy the adobe CS2 suite and build it as a dynamic PDF. You can create buttons that will submit it in a variety of formats.
